These are the forms that are required for enrollment. You must also include a copy of each child's immunization records.
Please open the form that you need, print it, fill it out completely, and bring it with you to register your children at Kiddie Kare. Each child MUST have their own set of forms.

Employment Opportunities
Kiddie Kare of Arizona is always accepting applications for employment. Please look at our list of requirements, and if you think you have what it takes to be a child care professional within our organization, please fill out the application below and submit it to us.
We offer tuition discounts and advanced educational opportunities to ALL our employees.
Requirements:
- Must be 18 years of age.
- Must have a GED or a high school diploma.
- Must have, or be able to obtain, a fingerprint clearance card through the state of Arizona.
- Must have, or be able to obtain, a negative TB test.
- Must be able to pass the DCS background check.
- Be willing to attend college classes in early childhood education.
- Must be child-friendly and customer service-oriented.
- Must be able to lift 20 lb. or more and move freely throughout the classroom.
- Use of household chemicals is required.
